Please Note: If you cannot find an in-person appointment in a timely manner, you may select the virtual appointment option from the menu on the left and the virtual only appointment openings will show up.

New Patients:

Returning Patients: (please choose carefully)

1.) If this is our second visit together: schedule a Second Appointment (virtual or in-person). During this appointment, we will review your lab work and I will disclose your treatment plan for the next 3-months.

2.) All subsequent follow-ups are booked as Standard Follow-up (virtual or in-person) with a maximum of 45 minutes. If you need more time, you can also book an Extended Follow-up (virtual or in-person).

3.) If you have a new concern or have not been in to see Dr. Erin in over 6 months, please book an Extended Follow-up (virtual or in-person).

4.) If you have not been in to see Dr. Erin in 2 years+, please book an Initial Appointment (virtual or in-person) as your concerns will need to be re-evaluated with new treatment recommendations.
